Why would they be interested in all of this information to apply for a birth certificate? If anyone knows, please inform me.

The only reason I can come up with is that maybe it effects the value of a bond that may be written on the birth certificate and it's value may depend on the desease history of the child which effects future potentials earnings. The value of the bond written may depend on wheather the child is deaf, Down's Syndrome, healthy etc.
